As a new hire, you may elect some coverage without answering health questions, known as the Guarantee Issue (GI). The GI amounts are as follows:
Should you elect an amount that exceeds the Guarantee Issue, an Evidence of Insurability (EOI) form will be required for underwriting review. You will not be charged for an amount that exceeds the Guarantee Issue unless you are approved.
During enrollment, you must review your life insurance beneficiary. The beneficiary is the person you designate to receive your life insurance benefits upon your passing. Your beneficiary can be a person or multiple people, a charitable institution, or your estate. Once named, your beneficiary remains on file until you make a change.
Coverage continuation options are available in the event of employment separation. If you wish to continue your coverage upon separation, you must contact Voya Financial within 30 days of separation. You must elect portability coverage before age 70, and it terminates at age 80. Portability rates differ from active employee rates.
